Reflect on the information you have reviewed this week on Community health model or theory. Write a short paper in which you:

1) choose a community and define them and their health problem.

2) choose a community health model and/or theory,

3) describe the model and why you have chosen it.

4) Explain how the Community or theory could be applied to the community/health problem you chose. 5) Then draw or create a computer graphic of the Community health theory applied to the health problem.
