Mental Health Shadow Health Tina Jones Transcript

The conversation transcript with Tina Jones about her mental health is as follows:

Hi, my name is xxxx. Today I’ll talk with you and later check you over.

Hey there.

What’s your name?

I’m Tina Jones.

Why did you come to the clinic today?

I’m here because I’ve been having a lot of trouble sleeping… I feel more nervous lately, especially before bed.

I’m sorry to hear about your sleep troubles and nervousness. I’m glad you came. I’ll look into it further.

Thanks for saying that.

How long have you had these symptoms?

Well… I’ve known I have a lot coming up for a while, so I was worried about stuff starting a few months ago. But the stress and sleeping problems really started about three and a half weeks ago.

How often do you feel stressed and have trouble sleeping?

Before, I’d only get nervous before a test or something, but now it’s every night before bed.

I’m sorry to hear that, Ms. Jones. We’ll explore this more to find the root cause.

Thanks for understanding.

What makes your stress and sleep problems worse?

When I need to sleep, I start worrying that I won’t be able to. Then I start thinking about graduating, the CPA exam, finding a job, moving out of my mom’s… everything piles up when I have time to think about it.

I get it. We’ll learn more as we continue the interview.

I appreciate you telling me that.

Do you sleep during the day?

No, I wake up two or three times a night for at least ten minutes.

Do you drink caffeine at night?

I know caffeine can make it hard to sleep, so I only drink it in the morning or early afternoon. Never after 4 PM.

That’s considerate.

Do you drink alcohol before bed?

I haven’t been drinking lately. I have too much to do.

It sounds like you have a lot on your plate. I’m sorry you’re dealing with so much.

Thanks for understanding.

Do you take any drugs before bed?

No, I don’t do drugs.

Do you have any breathing problems affecting your sleep?

Yes, I have asthma.

I’m sorry to hear about your asthma.

Could asthma be affecting your sleep?

Could asthma be interfering with your sleep?

What relieves your stress and sleep problems?

My sleep issues aren’t related to breathing. Since I started using my inhaler daily, I haven’t had breathing problems.

What helps you feel less stressed?

I usually try to stay organized, make to-do lists, plan my time. Sometimes talking to friends or family helps. Going to church usually helps too. But lately, none of it is really working like it should.

It’s good to have a strong support system.

I appreciate you saying that.

What helps with your sleep problems?

I just feel really nervous before bed. I start worrying about things and stressing about the future, and my thoughts start racing.

I’m sorry to hear that you’re so nervous.

Thanks for understanding.

Do you exercise?

I don’t have time to exercise, honestly.

How have your sleep problems affected your daily life?

No.

Have you been diagnosed with obsessive-compulsive disorder?

No, I’m lucky I haven’t had to deal with that.

Have you ever taken psychotropic medication?

No, thankfully I’ve never needed that.

Do you have depressive symptoms?

No, I don’t think so.

Do you have difficulty making decisions?

Everything is a little harder because I’m out of it from not sleeping.

I’m sorry to hear that.

Are you feeling sad?

I’m not really sad, just stressed out.

Any loss of appetite?

I’m less hungry lately. Not sleeping is making me feel queasy during the day.

Have you ever thought about suicide?

No, never.

Have you ever attempted suicide?

No, I turn to God in tough times.

Any self-harm?

I don’t hurt myself, like cutting. I’ve never done that.

It’s good to hear that.

Do you have a past trauma?

Not really.

Do you sometimes have angry outbursts?

I don’t get angry a lot. Being patient with people makes everything easier.

That’s true. Sometimes anger can indicate mental disturbance.

Okay, thanks for letting me know.

Have you ever been violent in the past?

No, never.

Are you currently employed?

I’m a supervisor at Mid-American Copy & Ship. I’ve been there since high school.

That’s good to have a job.

Thanks for saying that.

How many hours do you work per week?

About thirty-two hours a week right now.

That’s good. You’re managing both work and education.

Thanks.

I do sometimes, yeah, when I go out with my friends.

How often do you drink alcohol?

Once or twice a week, sometimes less.

Excessive alcohol is harmful to your health.

Sure, I think I understand what you mean.

Do you drink and drive?

We usually take a cab when we go out.

That’s good. Driving under the influence of alcohol is dangerous.

Okay, thanks for letting me know.

Do you take any prescription medication?

Just Proventil and Flovent inhalers for my asthma.

Do you misuse prescription drugs?

No, I wouldn’t want to take someone else’s prescriptions.

That’s great. Some people use prescription drugs as recreational drugs.

That makes sense.

Are you in a relationship?

No, I’m single.

When was your last relationship?

About three years ago.

Have you ever been married?

Nope.

Are you sexually active?

Not lately.

Do you use condoms during sex?

No, I’m single now, but before I was on the pill.

Using protection during sex is important.

Okay, I understand.

Have you ever been abused?

No, never.

What do you think is causing your stress?

I’m stressed because I can’t sleep, but I can’t sleep because I keep worrying about graduating, passing exams, finding a job, and my living situation. It’s a lot.

I’m sorry you’re dealing with so much.

I appreciate you saying that.

How do you cope with stress?

I usually try to stay organized, make to-do lists, plan my time. Sometimes talking to friends or family helps. Going to church usually helps too. But lately, none of it is really working like it should.

That’s great. Social networks can help with stress.

Sure, I think I understand what you mean.

Is there any family history of mental illness?

No, nobody’s been diagnosed, but they can be a bit crazy.

Any family history of addiction?

My uncle, my dad’s brother, has an alcohol problem, but we don’t see him anymore.

Any family history of sleep problems?

I don’t think so, just me.

Have you had sleep problems in the past?

After my dad died, I had sleep issues, but not as bad as now.

How often do you have sleep problems?

Most nights, I’d say. Four, sometimes five nights a week.

I’m sorry to hear that. Sleeplessness can be tough. We’ll see how we can address this.

Thanks for understanding.
