Palliative care and hospice care are specialized care that are essential to not only to geriatrics but also to people suffering from chronic conditions. According to the Center for Disease Control and Prevention, chronic diseases such as heart conditions and cancer are the leading cause of mortality in the United States. Approximately 86% of funds allocated to the healthcare sector are spend each year in managing this conditions and their complications. Many patients receive general care like other acute patients when they would have benefited better if they received palliative care or hospice care depending on the stage of the disease (Center to Advance Palliative Care | CAPC, 2016)…

In nursing homes, assisted living residences and hospitals where palliative care and hospice services are offered, lack of knowledge on assessment of symptoms, management and prognostication costs patients (Swami & Case, 2018).)…

High level of detail regarding the problem

Difference between palliative care and hospice care that is misunderstood by many lies on the goals of care and the prognosis. While palliative care is offered at any stage of life, hospice care is provided at end stages of life; when a patient remains with less than six months to live if the disease had to follow its natural cause. The primary goal of both services is to improve quality of life, manage symptoms and pain ensuring the comfort of the patient and the family (Hospice Vs. Palliative Care, 2016)…

Impact of the problem/issueon the quality of care and patient outcome

Lack of knowledge and skills among care providers on palliative and hospice care especially on communication, assessment of symptoms, other needs and management affects the quality of care and quality of life of the patients (Eide Lunde, 2017). Some of the urgent needs of the patients and the families go unattended as the healthcare practitioners, attending palliative and advanced stage patients, concentrate on the physical symptoms and management of pain mostly. Neglecting of functionality, social and spiritual support to chronic patients by nurses and other attendants shortens survival of those patients. patient and family  satisfaction and comfort which are the main aims of palliative care are not achieved.
