Mrs. X, having been diagnosed with acute renal failure at the age of 55 years, is in the preparation stage of the transtheoretical model. This stage involves the readiness of the client to take actions to enhance health (Tang &Fu, 2021). The renal nurse ensures that Mrs. X enhances positive behavior change to limit further damage and promote quality of life. It is achieved by using the nursing process, which involves five stages: assessment, diagnosis, planning, implementation, and evaluation. This process ensures that the care provided is patient-centered, holistic, and goal-oriented.

Assessment

This step entails collection, organization, validation of objective and subject data. Subjective data is the information that the patient reports experiencing, while objective data is observed by the nurse (Doenges et al., 2019). The nurse should conduct a comprehensive health history and head-to-toe physical exam to identify edema or other disorders. Ask about diet and fluid restrictions. Diagnosis

A clinical judgment concerning an individual’s, family’s, or community’s response to current or future health concerns is defined as a nursing diagnosis.. Mrs. X’s diagnosis is fluid volume excess related to fluid retention secondary to the compromised regulatory mechanism, as evidenced by elevated blood pressure, edema, fatigue, and jugular vein distention (Carpenito, 2021). Patient experiences actual problems because they have not started to act on the recommended interventions but are ready.

Planning

The goal is for the client to display actions that will allow them to monitor their body’s fluid state and reduce the occurrence of fluid excess and electrolyte imbalance. The outcome is that the patient will exhibit normal ranges of vital signs, especially blood pressure, absence of edema, and a balanced input/output chart (Hinkle et al., 2018). The degree of edema level should assed and location documented. Vital signs should be monitored to obtain baseline data and allow early intervention to be carried out.  .

Implementation

The nurse’s nursing interventions to be carried out include electrolyte imbalances, fluid management, and monitoring. Because it’s vital to determine the renal function fluid replacement needs and prevent the risk of fluid overload, the nurse should carefully record the input/output chart (Carpenito, 2021). The patient is educated on adhering to dietary restrictions such as decreasing salt intake, planning an appropriate menu, and drug compliance. The client is also encouraged to elevate the legs whenever possible and limit crossing ankles or legs. In addition, significance of nutrition to the treatment process is taught.

Evaluation

This is the last step of the nursing process, where the nurse determines if the client exhibits any behavioral changes and if the interventions are working. The client demonstrated the ability to record fluid intake/output, adhered to dietary restrictions by limiting salt intake, and understood the need for elevating legs and avoiding crossing ankles (Doenges et al., 2019). Vital signs returned to normal, electrolyte imbalance restored, and edema was absent. Therefore the goal was fully achieved. Significant others, such as spouses and adult children, should be included in treatment programs to promote a healthier lifestyle.
