PROJECT TOPIC: supporting new graduate throughout their transitioning and beyond.
Presentation: The Theory or Conceptual Framework and Change Model Used to Support the Quality Improvement Project
Now you have to create a synergy for a collaborative interprofessional effort to effect change.
To do this, you have to engage the stakeholders of the project and this includes the interprofessional team. You need to create buy-in from the team by helping them understand how this will happen and how they will collaborate in the process. They need to have a clear vision of the theory or conceptual framework behind your ideas related to best practice and the model for change you want to use in integrating and implementing the interventions into the model.
For this assignment, you will complete a PowerPoint presentation with voiceover (using 10-12 slides, not including the introduction and reference slides) that is being delivered to the interprofessional stakeholder team. This team will help you develop the project. Be clear about the theory or conceptual framework using terminology that the team will understand. Then focus on the model for change and why it is a good fit for this project. Next, give detail about the steps in the process and how the steps are actually integrated into the model for change. End with questions that give the stakeholders a sense that you want their feedback and collaborative presence for this project. They need to understand that they will help support the success of the project. Use a title and reference slides.
