Effectiveness of My Change
Introduction
Evidenced Based Practice enables provision of quality and affordable care to the patients. This is accomplished by employing best practices, which have been tested and approved. My EBP project involves reduction of the rate of hospital readmission when transferring patients from one hospital to another through the transition care programs such as Project BOOST as well as transition care model(Aarons et al., 2014). The model facilitates effective and efficient transfer of the needed information when transferring patients from one hospital settings to another. Application of such model will facilitate reduction of readmission rate for patients aged 55 years.
Monitoring the patient outcome is one of the ways of determining the effectiveness of my EBP. This can be accomplished through monitoring and evaluating the implementation of my EBP vs. the patient’s outcome within the given time (Ilic et al., 2013). In this case, healthcare professionals are supposed to apply the model when transferring the patients from one department to another. Importantly, monitoring the patient rate of readmission when the models are employed is the most important aspect (Ilic et al., 2013).
The objective of my EBP is to enable reduction of the readmission when the transfer models are used. I will achieve this by evaluating the rates of patient readmissions in facilities where my EBP has already been applied, in comparison to other where the EBP was not implemented. Monitoring the implementation and the outcome of the EBP shall also help in determining the effectiveness of my EBP. According to Ruzafa-Martínez (2016), it is important to keep a closer look at fidelity of the implementation of EBP to determine its effectiveness to the study population. My EBP shall be implemented effectively, to make sure it guarantees positive health outcomes. It is, therefore, important to consider the implementation process of the EBP to determine its effectiveness (Aarons et al., 2014). Another way is following the customer or client response or feedback, which shall be used to gauge how the clients felt about the process, and how they felt when their medical history was well updated in all facilities they attended. Patients are more satisfied when quality care is provided to them.
Conclusion
The science of translating EBP into practice is still finding its ground in the healthcare industry. As a results, there is need to implement proper, effective, as well as efficient strategies, aimed at ensuring that effectiveness of EBPs are determined. Some of the strategies include; following the EBP fidelity implementation, comparison of the implementation versus the patient’s heath outcome and checking the patients’ feedback as suggested by this research.
