Nursing plays a vital role in the medical field in ensuring the quality outcome of patients. On top of being the backbone of the healthcare system, practitioners in the nursing field are in the frontline in transformations that are occurring in the healthcare sector. Services rendered by nurses have an impact on the patients, their families and even to the nurses themselves.

In the ambulatory clinic, a geriatric patient presented with general malaise and blurred vision. The patient had wasted a lot. The daughter who accompanied him stated that the father had lost more than approximately 5 kilograms of total body weight in that year. The patient experienced progressive difficulties to ambulate.
The family was appreciative too for being enlightened on the importance of providing care per the patient’s expectations. Caring for their patient according to his values and preferences led to quality outcomes.
Patient-Centered Care.
Nurses as caregivers focus on giving holistic care to the patients. To ensure quality care achieved, the nurses focus on the patients as individuals despite the condition they are suffering from. Under the guidance of the primary nurse, I treated each patient uniquely with dignity concentrate on the patient’s prevalence and values.
We ensured that the patients understood the condition they were suffering from, the prognosis and progress of care to alleviate the anxiety in the individual patients. Through giving health education to the patients, nurses improve self-care among the patients they attend to and boost autonomy of the patients.
Teamwork and Collaboration.
As a result of specializations in the medical field in general and nursing field in specific, there is a need for inter-professional care to ensure patients receive comprehensive care (Ratzliff et al., 2016). Most of the patients I handled while in the clinical setting, especially the geriatric patients needed inter-disciplinary attention. I understood that nursing services are the center of all the care the other professional such as nutritionist, physician and physiotherapist offer. We linked up with other caregivers in giving care and making decisions concerning the diagnostic results and progress of patients. Communication is the most potent weapon used by the various caregivers to coordinate the care. Enhanced communication in teamwork in health care brings about collaborative care improving the performance and patient outcome. Making decisions together and working as a group also minimizes errors among clinical care providers. The team working on the other side benefits the caregivers even more. Teamwork makes work easy hence nurses who believe and engage in collaboration get more satisfied with their work.
Evidence-Based Practice.
Evidence-based practice is one of the trending clinical practices in line with inter-disciplinary care. Nursing practices in the clinical settings and the communities are based on evidence from research and clinical experience (DiCenso, Guyatt, &Ciliska, 2014).
Safety and Quality Improvement.
We ensured patient safety and quality care through the use of the electronic health records in transferring patient’s information from one unit to another. Use of electronic records reduced the time we spent going to the laboratory department to collect patients’ diagnostic results.

Conclusion
Clinical experience makes the theory learned in the class setting realistic. Application of the theory into the real-life situation boosts understanding. From the clinical experience, I was able to identify what theory was applicable.
